WHAT IS YOUR APPROACH TO PRESCRIPTIONS?

(Constitutional, Causations, Pathology, Key notes, Mental only, Physicals only, Repertory, Miasmatic, Nosodes, Bowel nosodes, Biochemic remedies, Bach flower remedies, etc.)

“My favorite approach to prescribing is ‘Keynotes’, which includes causation, pathology and a few mentals. In a few cases, my prescriptions are based on the constitution and miasm where the mentals play a great role. Apart from these, from time to time, I may give nosodes or bowel nosodes. This is the best approach for me as it has given the maximum results. ”

- Dr. Neeraj O. Goel, Professor and Head of Materia Medica Dept., CMP Homoeopathic Medical College, Bombay, India

“Approach to cases depends on the type of case. I have no set rule or fixity. Practically, the constitutional/causation and keynotes/pathological approach works for most cases depending on their presentation.”

- Dr. S. K. Goel, Principal of CMP Homoeopathic Medical College, Bombay; formerly A.M.O. Govt. Homoeopathic Hospital, Bombay and Deputy Director- Homoeopathy, Govt. of Maharashtra

“Every case demands a different method (of approach).”

“There is a never a favorite approach. Each case presents a different angle and after the study, which angle to follow, is decided. There is no favorite approach.”

Except Bach flower and Biochemic, all other aspects are considered. The totality of symptoms under different aspects is considered to make a prescription. The various aspects that construct a total picture of the disease helps me in simplifying the process of understanding cases. The remedy is chosen after tracing the connection and determining where lies the weight in the case but at the same time it is also important what goes through and through the case, e.g.

A 28 years old male, Mr. RA, consulted me in January 1996 for recurring cold and bronchitis. He complained that in last 4 years I have been having frequent throat infections, which starts with sneezing, later bodyache and fever. Inspite of my leaving cigarettes the problem still continues and has aggravated to such an extent, that it comes every 3 months. I cannot breathe. I start wheezing. Constant feeling of tiredness throughout the day.

When I asked him to talk about himself - he said I am into computer software.  My line is sales and communication. I love meeting people and love meeting friends. I got into it because it is not monotonous and everyday there is something new. I love traveling. I work 12-14 hrs/day regularly for 7 days. Now body feels tired and heavy. Never given importance to my body and looking after it.  I like taking up challenges.  If something has to be done in two weeks I have to do in 4 days. I know people suffer because of me. Since I never give time to my parents and wife.

My motivation is that I have to come up with ideas. If there is a problem you have to come up with 10 ideas. If something is not possible through route A then you have to try 2nd or 3rd route. There is always an opening. I love fast driving. I eat 6-8 bananas in the morning.

Prefer windows open. I feel the fresh breeze or else I feel suffocated. I have a nightmare, never to get stuck in a small hole where the ceiling is short where I cannot stand up to my full height making me bend all the time. I cannot be in such a place for more than 5 minutes.  Before that I have to “BREAK AWAY, GET OUT”. No dreams.

What was most apparent was the need for very intense, rapid and constantly new or changing activity. Glimpse of the inner feeling is too much work being compressed in too little time and need to find an opening. Other features of case like need for traveling, love for fast traveling, intolerance of closed space on be explained by his main feeling.

His main feeling is of being cramped into narrow a place or being oppressed. His reaction is, wanting to break out and this can be achieved only by rapid activity which leads to exhaustion.  This feeling is seen, as well, in his physical complaint, which is an inability to breathe.

The remedy chosen was Bacillinum, which is known to affect the lungs and cause difficulty in breathing.
